Okay, a lot of people didn’t like the Barney The Dino comparison of Covid being less lethal than anyone predicted. But now that it’s proving itself to be even less lethal then the ones who were already saying it wasn’t lethal in the first place, this is a perfectly timed post. I’d be more concerned about the seasonal flu if I even thought Germ Theory made sense that we get sick because of germs. 𝐋𝐞𝐭’𝐬 𝐭𝐚𝐥𝐤 𝐚𝐛𝐨𝐮𝐭 𝐡𝐨𝐬𝐩𝐢𝐭𝐚𝐥 𝐫𝐨𝐮𝐧𝐝𝐬!⁣⁣⁣⁣🏥⁣ ⁣⁣ For the majority of my surgery rotation I was rounding on post-operative hospital patients. I wish someone had taught me how to conduct presentations for rounds. Here is a guide to organize your patient presentations for your preceptor.Presentations are often organized in a SOAP (Subjective, Objective, Assessment, & Plan ) note format.⁣⁣⁣⁣⁣ ⁣⁣⁣ I like to start off with the patient's name and age followed by the surgery performed, indication, and date of surgery⁣⁣⁣⁣⁣ ⁣⁣⁣ 🔶𝐒𝐮𝐛𝐣𝐞𝐜𝐭𝐢𝐯𝐞 - includes patient complaints and nursing notes. It can include things such as appetite and pain, but it’s most important to list any changes or new symptoms.⁣⁣⁣⁣⁣ ⁣⁣⁣ 🔶𝐎𝐛𝐣𝐞𝐜𝐭𝐢𝐯𝐞 - includes info that’s measured and not open to interpretation:⁣⁣⁣⁣⁣     🔹𝘝𝘪𝘵𝘢𝘭𝘴: Temperature, blood pressure, heart rate, respiratory rate, oxygen saturation. If a patient is on supplemental oxygen make sure to include the liters per minute. If they are on ventilation or high flow nasal cannula include the FiO2.⁣⁣⁣⁣⁣     🔹𝘐𝘯𝘴/𝘖𝘶𝘵𝘴: Includes what is going into the patient and what’s leaving the patient. For example, IV fluids (include rate ), PO fluids, patient diet (including NG tube feeding, TPN, or PPN ), urine output, output of all types of drains, and bowel movements.⁣⁣⁣⁣⁣     🔹𝘗𝘩𝘺𝘴𝘪𝘤𝘢𝘭 𝘌𝘹𝘢𝘮: I always listen to heart and lungs no matter what surgery was performed, but also be sure to perform a focused physical exam based on what surgery was done. Convey all positive findings and pertinent negatives in your presentation.⁣⁣⁣⁣⁣     🔹𝘔𝘦𝘥𝘴: List any meds the patient is taking in the hospital, including their home meds, if they are relevant to the case.⁣⁣⁣⁣⁣     🔹𝘚𝘵𝘶𝘥𝘪𝘦𝘴: Check all labs that are provided, such as cultures, BMP, and CBC. I like to also trend procalcitonin, ESR, and CRP as they are a non-specific marker for inflammation. Also be sure to include the results of any imaging performed.⁣⁣⁣⁣⁣ ⁣⁣⁣ 🔶𝐀𝐬𝐬𝐞𝐬𝐬𝐦𝐞𝐧𝐭 & 𝐏𝐥𝐚𝐧 - includes the patient’s diagnoses, and treatment plans. Have a plan for each diagnosis including therapeutics, diagnostic studies (labs, imaging, etc ), patient/family education, referrals/consults, and follow-up time.
