This is a story in the brand new series "Night shift" This is a story of my very first night shift. Enjoy! ~~~ ~~~ It was in 300 level 1st semester that I had my first night shift, the 2nd time I was posted to Male Medical Ward (home ). I was one of the last people in my class to be on night shift, we were lucky but night shift is inevitable in Nursing so there I was that night for the shift from 8pm-8am. I tried my best but I couldn't sleep properly in the day so by to 9 I was already tired and I had 11 hours to go, I was sad. After serving medications and doing other nursing procedures, it was to 11 and oh my goodness I was tayad!! and I wanted to sleep so badly. My body was not yet familiar with night shift mode and I still had 9 hours to go. The Nurse I was working with and the 3 student nurses were looking so bright eyed while I was looking like hell. When I told the Nurse it was my first night shift, she laughed and oriented me about night shift. She said ideally you should not sleep at all during the shift, but you and your colleague can take turns sleeping. You just gotta have that stay awake, get the job done, the shift will soon be over type of vibe. That really clicked in my head. Then she asked who wants to sleep and the way I said me quickly, everyone laughed and let me sleep first. ~~~ ~~~ I must say for my first night shift, I was lucky. Male Medical Ward has an amazing Nurses' room. I wore my sweater and slept in this really comfy chair and I had the privilege of saying the AC was too much. I was given 2 hours, so by 2am I had to wake up. I was bright eyed and I could work. In between replacing intravenous fluids and checking on patients, there was nothing to do, it was kinda boring. So I sat at the Nurses' table using my phone, I was even texting my friend who lives in a different time zone so that kept me busy. By 3am I was hungry, so I made cornflakes with cold water, it was glorious. Then the shift just started to dragggggg, from 3am to 4am took forever, 5 mins felt like 30. Hoje, no Brasil, há mais de 13 milhões de pessoas vivendo com diabetes, o que representa 6,9% da população. E esse número está crescendo. Em alguns casos, o diagnóstico demora, favorecendo o aparecimento de complicações. Mas afinal, o que é a Diabete Mellitus? 🤔 É uma doença do metabolismo da glicose causada pela falta ou má absorção de insulina que é um hormônio produzido pelo pâncreas e tem como função quebrar as moléculas de glicose para transformá-las em energia, a fim de que seja aproveitada por todas as células. Diversas condições podem levar ao diabetes, porém a grande maioria dos casos está dividida em três grupos: DM tipo 1, DM tipo 2 e DM gestacional. De acordo com a Federação Internacional da Diabetes (IDF ), para a maior parte das pessoas que têm diabetes, os valores alvo de glicemia são os seguintes: 🟣Em jejum (glicemia plasmática em jejum ): menos de 115 mg/dL 🟣2h após as refeições (glicemia pós-prandial ): menos de 160 mg/dL 🟣Não deve ter valores inferiores a 70 mg/dL em nenhuma altura do dia. Quando os valores estão abaixo de 70 mg/dL, falamos de hipoglicemia ou "baixa de açúcar", uma situação que pode ser perigosa e é de evitar. ⚠️A hiperglicemia é a elevação das taxas de açúcar no sangue e que deve ser controlada. Sabe-se que a hiperglicemia crônica através dos anos está associada a lesões da microcirculação, lesando e prejudicando o funcionamento de vários órgãos como os rins, os olhos, os nervos e o coração. Os pacientes que conseguem manter um bom controle da glicemia têm uma importante redução no risco de desenvolver tais complicações como já ficou demonstrado em vários estudos científicos.⚠️ ✅Lembrando que é muito importante ter um acompanhamento médico, onde será possível rever o caso e fazer algumas alterações sua dieta, rotina, atividade física ou medicação. How do you know when it’s time to move on from your current job role? Have you felt these things before? There are definitely a lot more signs but these are some of my personal main ones. Are there other signs I might have missed?☝🏾I have had four jobs in six years 🤣 and I have definitely felt all of these emotions at some point ... normally at the point when it was time to move on. This is just a gentle reminder to whoever may need it that you shouldn’t be scared to move on or try new things. As I always tell myself if something doesn’t work out there will be plenty of other opportunities that come your way. It’s also worth noting that moving on doesn’t necessarily mean you have to leave your Trust or ward, it can be as simple as going into a different sub speciality within your current unit or going on a secondment. Cardiogenic Shock is a condition in which your heart suddenly can't pump enough blood to meet your body's needs. The condition is most often caused by a severe heart attack. Not everyone who has a heart attack has cardiogenic shock. Can be due to an MI, PE, heart arrhythmias, valve problems. You might think you want a hospital job, but after shadowing you might find that the lack of flexibility and/ or call schedule might be too much for your personality OR you might find a private practice has too much entrepreneurial involvement than what you might have envisioned. Start the search early! ⠀ ✔️Know your value and bring it to the table. I can’t stress this enough! Do your research on compensation in your speciality and the region ✔️OPEN the door for your position. If you find your “dream” job and there’s not a position open, then MAKE IT HAPPEN. Cardiology IV vs Classic 3 stethoscopes 🩺 . Now that I’ve had the chance to use both, I can safely say that the cardiology @3mlittmann is my all time favorite! The sound quality is absolutely out of this world! . A couple things that I saw when I googled classic vs cardiology before buying included people saying the cardiology is much heavier, the sound quality is not noticeable, and the price doesn’t match the product. I have to say that I disagree with all three of these. It feels the same around my neck/in my pocket (maybe that’s just me! ), and I feel like I can hear things now that I never would have heard with my classic 3, especially on the babies. For me it was definitely worth the upgrade! . What kind of stethoscope do you use?

As a Geriatric Psychiatry Nurse Practitioner, the impact of COVID-19 has been life-changing. #ad My usual week consists of visiting multiple long term care facilities to provide services, however, in a effort to protect my patients I have been limited in my abilities. I really miss the one on one contact, but I have found Telehealth to be the next best thing for my practice! As we begin to reimagine what opening up looks like for our country, it is important to note the special place that essential workers have in this conversation. We need to protect them, as we begin to move back to this one on one contact we all are looking forward to. Before we open up, we should hold our government accountable for its shortcomings. If they had handled this situation better from the start, the problems likely would have never gotten this out of hand. One of my favorite things about ophthalmology is that it's a surgical specialty! Even though I spent some time in the OR as a first year resident, I'll be spending a lot more time there as a second year resident (including having the opportunity to be the primary surgeon on cataract cases ). Like developing any other skill, practice is necessary! I've been putting in time in the wetlab to practice the different steps of cataract surgery 👁️. Unrelated but I'm impressed with my posture. 😂

.⁠⠀ 🩺 How “hard” is Nursing School 🩺 ⁠⠀ .⁠⠀ Like any other professional schooling (business, med, education, engineering, etc. ), there is a level of intrinsic adjustments that need to be acknowledged and act upon. ⁠⠀ .⁠⠀ Yes, nursing school could be considered hard; but hard is relative. Most of my peers found or assumed that medical surgical nursing along with pharmacology and pediatric nursing is hard; whereas I found writing research papers for EBP hard. ⁠⠀ .⁠⠀ Nursing school is a lot of work. You learn something new everyday at a constant pace and you’re required to stay on top of your readings, assignments, papers, clinical, didactic etc. Not to mention the abundance of paperwork needed for each clinical site, and mastery exams. For my program, we have to received certification at four of our main local hospitals. Which excludes rehabilitation centers. ⁠⠀ .⁠⠀ The adjustments I mentioned earlier isn’t unrealistic. I’ve managed to work full time during my first semester, hang out with friends and family on the weekend, and I had my lazy days. Life is a balance. I passed L1 with a 3.49 GPA. It’s doable. ⁠⠀ .⁠⠀ Here’s the trick:⁠⠀ 1️⃣stay on top of EVERYTHING. Trust me it’s pays off in the long run. ⁠⠀ 2️⃣be very organized. You don’t have to invest in an elaborate planner; I didn’t, but that works for some people. I have a business background so I was already organized being an auditor and an accounting assistant. ⁠⠀ 3️⃣accept your limits. You’re not perfect, and I struggle with this myself. My family always remind me that it’s ok to have failed that one test; it doesn’t define what nurse you’ll be. ⁠⠀ 4️⃣ask for help. USE YOUR INSTRUCTORS. They’re getting paid to teach you. Office hours is there for a reason. Have a peer reexplain a concept in his/her own words. Maybe that would help you. ⁠⠀ 5️⃣finally, figure out how to study. You’ll quickly realize that the way you studied before might not work while you’re in nursing school. Also, figure out what studying is to you. For me, reading is studying, taking notes is studying, listening to khan academy is studying, everything is studying. Don’t limit yourself to a desk and a computer screen. ⁠
